Skilled Nursing Care

Heartland Care Center provides 24-hour care for 98 residents.  Both long-term and short-term needs are serviced. 

There are 51 private rooms and 23 semi-private rooms.  Specialized rooms available are: Comfort and Care Suite which is a private home-like setting where our residents and their families can be together while going through the difficulty of the dying process; a Rehab Suite, which is an all-inclusive Suite available for short-term Rehabilitation stays, and 2 Super-Suites, which are extra large “apartment” style rooms providing skilled nursing care, available for either an individual or a couple. 

Resident Life

At Heartland Care Center, we believe that a resident should have the opportunity to directly participate in selecting available options when it comes to their health care needs.  That is why Heartland Care Center offers its residents flexibility and personal choices to accommodate their lifestyle, involvement and amenity preferences.

Every resident is offered the opportunity to have direct input into their plan of care, which includes nursing, dining and activity programs.  In addition, they may be provided with other options, which allow them to maintain and exercise their autonomy and individuality. 

By recognizing a resident’s basic right to exercise a degree of control over their every day living situation, Heartland Care Center is taking an extra step to achieve its goal of providing residents with a high level of satisfaction. 

Quality Nursing Care

The quality care we provide is accomplished by staff who are trained to care for the individual needs of each resident. We provide care according to each resident's particular functioning level, ranging from early dementia to medically complex needs. Hospice services are available through Mercy Hospice.

Care of the resident is directed by an attending physician. Our local clinic has specific physicians who serve Heartland Care Center Residents or Residents may retain their own physicians if they prefer to travel to the clinic.

The Nursing Department is composed of Registered Nurses, Licensed Practical Nurses, Certified Medical Assistants, and Certified Nursing Assistants.  Each resident has a specific resident care plan, which specifies involvement in the various disciplines in each resident’s care.  The care plan is continually reviewed and updated to address the resident’s current medical and social status. 

North Dakota nursing home residents are classified into one of 34 levels of care on a quarterly basis. This classification is based on the intensity of services received, which determines daily rates by facility. Heartland Care Center, a skilled nursing facility, is certified for Medicare and Medicaid (Medical Assistance).

Dietary Services

A Licensed Registered Dietitian and Certified Dietary Manager plan and provide planned menus to provide optimum nutrition to meet the daily nutritional requirements of each resident.  Specialized diet plans are provided as appropriate.

Social Services

The Social Services Department facilitates admissions to Heartland Care Center, identifies the psychosocial needs of the residents, assists in helping residents achieve their highest potential and facilitates discharge planning.  Social Services staff works with all departments to develop a care plan to assist residents in achieving their goals, which often include returning to their home or lesser level of care following rehabilitation.

Therapeutic Recreation

The Activity Department provides activities such as cards, social gatherings, exercise groups, music, outings, crafts and computer usage (e-mail and internet usage).  Volunteers sponsor the beauty shop and birthday parties.  Through comprehensive, innovative programs, our aim is to assist individuals in achieving self-sufficiency and personal satisfaction.

Pastoral Care

Heartland Care Center has worship services weekly.  Devotion, Bible Studies and Rosary are held various time during the week.  Ministers from all Christian denominations are welcome and encouraged to visit.

Housekeeping, Maintenance and Laundry

We strive to provide a clean, beautiful and home-like atmosphere for our residents.  We provide laundry services for the facility as well as personal laundry services for all residents.  Your first impression is important to us.
